The first person you will meet will be the RN Care Manager for coordination of health care services.
Registered Nurse Care Management is included and complimentary for every service. An initial in-home care evaluation is conducted by the RN Care Manager before services start with a complete health care assessment and review of your health history. Information about mobility, safety, your likes and dislikes, what your concerns are, and more will be discussed.
At this time, you and your family will work together with the Care Manager to develop an easy-to-understand, goal-oriented Plan of Care. The Plan of Care is a combination of the Manager’s recommendations, yours and your family’s objectives, and the anticipated physician’s goal of treatment.
The Plan of Care and the assessment findings are reviewed for approval by your physician. You are provided a copy of the Plan of Care as well as having access to it through our on-line, secure portal. The plan is also given to the caregivers who will provide your services.
The Care Manager is involved in all aspects of care in order to facilitate communication and care between you, the caregivers, your family, and your physicians. Collaborating and communicating regularly with physicians allows for improved outcomes by detecting potential issues before they become serious.
